**Acknowledgment of Risk, Release of Liability, and Agreement to Participate**

I, (hereinafter referred to as \"Participant\"), desire to participate in Brazilian Jiu-Jitsu (BJJ) training, classes, and related activities at [Gym Name] (hereinafter referred to as \"Gym\"). I understand, acknowledge, and agree to the following:

1. **Inherent Risks:** I understand that BJJ is a full-contact martial art that involves grappling, submissions, and physical exertion. I acknowledge that there are inherent risks associated with BJJ training, including, but not limited to:

* Muscle strains, sprains, and tears

* Fractures and dislocations

* Cuts, abrasions, and bruises

* Concussions and other head injuries

* Neck and spinal injuries

* Paralysis

* Death

2. **Voluntary Participation:** I confirm that my participation in BJJ training at the Gym is voluntary. I am aware of the risks involved and freely choose to participate despite those risks.

3. **Physical Condition:** I represent that I am in good physical condition and have no medical condition that would prevent me from safely participating in BJJ training. I have consulted with my physician if I have any concerns about my ability to participate.

4. **Release and Waiver:** I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue the Gym, its owners, officers, directors, instructors, employees, agents, and volunteers (hereinafter referred to as \"Releasees\") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the Releasees or otherwise, while participating in BJJ training, classes, or related activities at the Gym, or while on the Gym's premises.

5. **Assumption of Responsibility:** I agree to assume full responsibility for any and all risks, injuries, or damages, known or unknown, which I might incur as a result of participating in BJJ training at the Gym.

6. **Rules and Instructions:** I agree to abide by all rules and instructions provided by the Gym and its instructors. I understand that failure to follow rules and instructions may increase the risk of injury.

7. **Medical Authorization:** I authorize the Gym and its representatives to obtain medical treatment for me if I am injured or become ill during BJJ training, and I agree to be responsible for the costs of such treatment.

8. **Photography/Videography Release:** I grant the Gym permission to use my likeness in photographs, videos, or other digital media for promotional or informational purposes, without any compensation to me.

9. **Severability:** If any provision of this waiver and release is held to be invalid or unenforceable, such provision shall be struck and the remaining provisions shall remain in full force and effect.

10. **Governing Law:** This waiver and release shall be governed by and construed in accordance with the laws of the State of [Your State].
