ASSUMPTION OF RISK AND RELEASE OF LIABILITY

PLEASE READ CAREFULLY BEFORE SIGNING

I, the undersigned participant, acknowledge that I am voluntarily participating in Brazilian Jiu-Jitsu (BJJ), martial arts training, fitness activities, and related programs offered by 1284 BJJ, its owners, operators, instructors, employees, and agents (collectively referred to as \"1284 BJJ\").

ACKNOWLEDGMENT OF RISKS

I understand and acknowledge that participation in Brazilian Jiu-Jitsu and martial arts training involves inherent risks, dangers, and hazards including but not limited to:

Physical contact with other participants

Bodily injury including cuts, bruises, sprains, strains, fractures, and dislocations

Serious injuries including but not limited to torn ligaments, broken bones, concussions, and other head injuries

Aggravation of pre-existing medical conditions

Risk of transmission of communicable diseases

Equipment failure or malfunction

Negligent or intentional acts of other participants

Death or permanent disability

I understand that these risks cannot be eliminated regardless of the care taken to avoid injuries.

VOLUNTARY PARTICIPATION

I acknowledge that my participation in all activities at 1284 BJJ is entirely voluntary. I have been advised to consult with a physician before beginning any exercise or martial arts program. I represent that I am in good physical condition and have no medical conditions that would prevent my safe participation in these activities.

RELEASE AND WAIVER OF LIABILITY

In consideration for being allowed to participate in activities at 1284 BJJ, I hereby:

RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE 1284 BJJ, its owners, operators, instructors, employees, agents, and other participants from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in such activities or while on the premises.

INDEMNIFICATION: I agree to indemnify and hold harmless 1284 BJJ from any loss, liability, damage, or cost they may incur arising out of or related to my participation in activities, whether caused by my negligence or otherwise.

MEDICAL TREATMENT AUTHORIZATION

I authorize 1284 BJJ to secure emergency medical treatment for me if necessary. I understand that I am solely responsible for all medical expenses incurred as a result of my participation.

RULES AND CONDUCT

I agree to:

Follow all safety rules and instructions provided by 1284 BJJ instructors

Inform instructors of any injuries or medical conditions

Maintain control of my actions and not engage in reckless or dangerous behavior

Respect other participants and maintain appropriate conduct

Use equipment properly and as instructed

MEDIA RELEASE

I grant 1284 BJJ permission to use my name, image, and likeness in photographs, videos, and other media for promotional purposes without compensation.

EQUIPMENT AND PERSONAL PROPERTY

I understand that 1284 BJJ is not responsible for lost, stolen, or damaged personal property. I assume full responsibility for my personal belongings.

SEVERABILITY

If any portion of this agreement is deemed invalid or unenforceable, the remaining portions shall remain in full force and effect.

GOVERNING LAW

This agreement shall be governed by the laws of [STATE/PROVINCE] and any disputes shall be resolved in the appropriate courts of [COUNTY/JURISDICTION].

ACKNOWLEDGMENT AND SIGNATURE

I HAVE READ THIS AGREEMENT CAREFULLY AND UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND 1284 BJJ. I SIGN IT OF MY OWN FREE WILL.
