RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND ASSUMPTION OF RISK

1. Assumption of Risk

I understand that Brazilian Jiu-Jitsu (BJJ), grappling, and martial arts training are high-intensity physical activities. I acknowledge that participation involves inherent risks, including but not limited to:

Scrapes, bruises, and skin infections (e.g., ringworm, staph).
Muscle strains, sprained joints, and broken bones.
Serious injury to the neck, spine, or head (including concussions).
In rare cases, permanent disability or death.

I voluntarily assume all risks associated with training, whether supervised or unsupervised.

2. Health and Safety

I certify that I am in good physical health and have no medical conditions that would prevent me from participating in martial arts training. I agree to:

Follow all gym safety rules and instructor commands.
Maintain high standards of personal hygiene to prevent the spread of infections.
Notify instructors immediately of any injury sustained during class.

3. Release of Liability

In consideration of being permitted to train at Iron Serpent Jiu-Jitsu I hereby release, waive, and discharge Iron Serpent Jiu-Jitsu, its owners, instructors, employees, and fellow students from any and all liability, claims, or causes of action arising out of my participation in classes, seminars, or competitions, including those caused by the negligence of the releasees.

4. Media Release

I grant Iron Serpent Jiu-Jitsu permission to use photographs or video footage of me taken during training for promotional and social media purposes.

5. Medical Treatment

In the event of an emergency, I authorize the staff of Iron Serpent Jiu-Jitsu to seek medical treatment on my behalf if I am unconscious or unable to do so, and I agree to be responsible for any costs incurred.

I HAVE READ THIS WAIVER AND FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT.
